CONSENT FORM FOR PROCESSING PERSONAL DATA

This consent form was prepared in accordance with the general data protection law. In accordance with article 5, section XII of Law 13,709, this document enables the free, informed and unequivocal manifestation, by which the holder/guardian agrees to the processing of his/her personal data and the data of the minor under his/her care, for the purposes determined below:
